POSITION SUMMARY:
- Assesses, plans, and provides physical therapy services to clients in their homes in accordance with Medicare, Arizona, and Federal regulations and guidelines.
- Contributes and participates in care coordination and patient-centered care as a part of an interdisciplinary clinical care team of nurses, therapists, CNA's and MSW's.

Qualifications:
- Professional Arizona PT license in good standing.
- Minimum of one year's experience with a preference for prior Home Health experience.
- CPR certification.
- Fingerprint clearance card.
- TB test result.
- Licensed driver with automobile insured in accordance with state and/or Agency requirements with a good driving record.
- Demonstrates strong verbal and written communication skills.
